The Claude Agent SDK does not automatically load plugins installed via `claude plugin install`. This adds plugin discovery from ~/.claude/plugins/cache/ and passes them as explicit `plugins` entries to ClaudeAgentOptions. Also adds "user" to setting_sources so user-level settings (including enabledPlugins) are honored. New env var CLAUDE_PLUGIN_DIRS allows specifying additional plugin paths.
